Does the authorization paper for a minor to get a pport expire?

My husband signed the authorization paper for his minor child to leave the country. Then her mother claimed she didn't get the pport in time and the paper expired. My husband never filled out another one but they still went on the trip. We are trying to understand what might have happened. Do these papers really expire if you don't use them right away?
